Musina Local Municipality is inviting formal quotations for the supply and delivery of stationery.

The formal quotation form must be used—no other forms will be accepted. You can obtain it through the following methods:

✅ Submission Requirements

Your submission must include the following documents:

• (i) Company Ownership Proof:

  • CIPC documents showing share ownership/directorship OR

  • A comprehensive CSD report indicating owner names, gender, race, age, disability status (with a medical certificate for verification), and company address.

• (ii) Completed MBD Forms:

  • The attached MBD forms (MBD 4, MBD 8, MBD 9, MBD 6.1) must be fully completed and submitted.

• (iii) Municipal Account:

  • A copy of the municipal account for both the company and its directors, not older than 90 days.

• (iv) Formal Quotation:

  • The completed formal quotation form.
